New Patients

Your first visit to Michael K. Block, DPM, LLC, establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. You can save some time at your first visit by enrolling in our patient portal and/or filling out the New Patient forms, located on this page.

Below is the link that will take you to our Patient Portal where you can set up an account. Prior to being able to use the patient portal, you will be asked to verify your email and then we will verify that you are a patient of ours. Once you receive notification that you can use the patient portal, you will be able to enter your demographics, insurance, medications, allergies, and you will be able to pay your bill. Once that information has been completed, please click on the "Send info" icon to transfer your information into your electronic medical record.

Although you have submitted information to your chart, there are still things that need to be filled out. On the New Patient tab in the portal, there are pdf fillable New Patient forms that you can fill out. Please complete and bring with you to your first appointment.

New Patient URL is below. Please click on it to create your patient portal which will enable you to fill out some of your demographic information and send that information to our office. Once our office receives your information, we will merge it into your electronic medical record.

We have enclosed the necessary documents for you to review and complete which are required to provide you with the best possible health care.  In addition to these completed forms, please arrive 15 minutes early to your first appointment so our office staff can input all of your pertinent data into your electronic medical file.  Also, please make sure to bring the following information with you to your appointment:

  1. Please fill out all forms included on this website and bring to your first appointment.
  2. Your insurance cards to make a copy for our records.
  3. The referral from your primary care doctor, if required by your insurance company.
  4. Any co-payment required by your insurance carrier.  Co-payments are due at time of your visit and may be paid in cash, check, or credit card.
  5. Photo identification, such as a driver's license.

The New Patient Forms are PDF fillable. You can fill out the forms on this website, print them out, and bring with you to your appointment. If you don't have access to a printer, you can save the New Patient PDF to your desktop and email it to
