Due to the rising cost of healthcare, we have updated our Patient Financial Responsibility Policy. Please review the information below so you understand your insurance benefits and payment responsibilities prior to your visit.

Insurance Deductibles
What is a deductible?
A deductible is the amount you must pay out-of-pocket for healthcare services before your insurance plan begins to contribute.
How it works:
For example, if your deductible is $1,500, you are responsible for paying eligible medical expenses in full until you reach that amount. Once your deductible is met, you and your insurance company begin sharing the cost of care through coinsurance.
On the day of your appointment, we will verify your current deductible status. Any remaining deductible amount will be due at check-in. We estimate your financial responsibility based on your insurance carrier’s contracted rates; however, these are estimates only and do not guarantee your final balance. If an overpayment occurs, it will be promptly refunded once your insurance claim has been fully processed.
Coinsurance
What is coinsurance?
Coinsurance is the percentage of costs you share with your insurance company after your deductible has been met.
How it works:
If your plan covers 70% of the allowed amount for a service, you are responsible for the remaining 30%. This percentage is your coinsurance.
We will verify your coinsurance responsibility on the date of service and collect the estimated portion due at that time. As with deductibles, these are estimates based on your insurance’s allowed amounts and may be adjusted after the claim is finalized. Any overpayment will be refunded promptly.


Co-Pays
What is a co-pay?
A co-pay is a fixed amount you pay at the time of your appointment. The amount varies depending on your insurance plan and whether you are seeing a specialist.
As a specialty provider, the applicable specialist co-pay will be collected at the time of service. You may be required to pay a co-pay regardless of whether your deductible has been met, and you may also owe coinsurance in addition to your co-pay depending on your specific plan.
Co-pay amounts are often listed on your insurance card or outlined in your insurance benefit documents. You may also access this information by logging into your insurance carrier’s member portal.
