Before Your First Visit

Your first appointment gives us the opportunity to get to know you, review your medical history, and understand your concerns so we can provide the best possible care.


To make your first visit smooth and efficient, we encourage you to set up your patient portal account before your appointment. Once you provide your email address, our team will send you an invitation to create your portal login.


Through the portal you can:

  • Enter your medical history, medications, allergies, and pharmacy information
  • Access your medical records anytime
  • Complete paperwork online
  • Pay your bill or co-pay ahead of time
  • Pre-check-in 24 hours before your visit using the mobile app


Tip: Including a list of your current medications and any known allergies in your portal helps our team prepare for your appointment.


If you haven’t activated your portal within 72 hours of receiving your email invite, simply click “Forgot Password” to request a new link.

What to Bring

For your appointment, please bring:


  • Your insurance card(s)
  • Your photo ID
  • A list of medications you are taking
  • Any referral forms from your primary care provider (if required by your insurance)



If you’re unsure whether your insurance requires a referral, call the number on the back of your card or contact your primary care physician before your visit.

Insurance & Payments

Any co-payment required by your insurance carrier is collected at the time of your visit. We accept cash, check, or credit card. You can also pay through the patient portal when completing your online check-in.
